A lung brother is a thing that nothing is more special than. It happens when one man donates his lung to another. Ray dreamed of being Roast Beef's lung brother, but Dr. Andretti ruined those dreams when he refused to harvest one of Ray's lungs because it wouldn't make sense. So Ray took it upon himself to shoot Doctor Kisses to donate a lung.
